The Battery That Doesn't Charge

A healthy person rests after exertion and recovers strength. Your loved one — does not. This phenomenon is called PEM (Post-Exertional Malaise). It's not just "fatigue." It's a systemic failure of energy production at the cellular level.

The Spoon Theory (Energy as Currency)

Imagine waking up with only 5 spoons of energy for the entire day.

  • Shower = 2 spoons
  • Getting dressed = 1 spoon
  • Making breakfast = 2 spoons
  • A walk = DEBT

Overzealous Bodyguard (MCAS)

Imagine your loved one has a Bodyguard in their body. His job is to protect against viruses. But this Bodyguard has gone rogue.

  • Food (Histamine) The Bodyguard thinks it's poison. Attacks the stomach, causes pain and brain fog.
  • Scents (Perfumes, Chemicals) The Bodyguard treats your perfume like tear gas. It can cause shortness of breath or fainting.
  • Stress & Emotions Even joy or an argument triggers chemicals that "set off" the Bodyguard.

It's not "all in their head." It's a biochemical reaction.

Enemy No. 1: Gravity (Why can't they stand?)

You may notice your loved one sitting on the floor in a store, squatting, or needing to lie down immediately. It's not a whim.

  • Circulation Issue (POTS): Blood pools in the legs and doesn't return to the brain. Standing still is like running a marathon for them.
  • Spine Issue (TCS): An upright position physically stretches the spinal cord. This causes back pain, leg pain, and bladder issues.

How to help?

  • Let them sit/squat anywhere (even on the sidewalk).
  • Don't force them to stand in lines.
  • Sit down next to them during a conversation.

More than fatigue - Symptom Map

Head & Nervous System

  • Brain Fog: Trouble finding words, memory issues, concentration.
  • Headaches: Chronic migraines, pressure in the back of the head.
  • Sensory Overload: Light hurts, sounds cause pain, touch irritates.
  • Dysautonomia: Dizziness upon standing, seeing spots/stars.

Pain & Muscles

  • "Migrating" Pain: Joints one day, muscles the next, skin the next.
  • Coat Hanger Pain: Heavy head feeling, stiff neck (common in TCS/CCI).
  • Paresthesia: Numbness, tingling, "electric shocks".

  • POTS: Heart racing like after a run, just from standing still.

Disease Spectrum

Mild (~25%)

~25% of Patients

Able to work (often with difficulty), but at the cost of personal life. "Dead battery" on weekends.

Moderate (~50%)

~50% of Patients

Housebound. Going to the store is an expedition requiring recovery. Naps are necessary.

Severe (~20%)

~20% of Patients

Bedbound. Dependent on care for washing/eating. Light and sound sensitivity.

Very Severe (~5%)

~5% of Patients

Totally bedbound. Often requires tube feeding. Extreme sensitivity to stimuli (must stay in darkness and silence). Every movement is a huge cost.

How to recognize their state?

Good Day

  • Can talk
  • Light walk (maybe)
  • Watch a movie together

Warning

  • Difficulty finding words
  • Light sensitivity
  • Needs a break!

CRASH

  • ZERO stimuli
  • Dark room
  • Don't ask, bring water

User Manual

IMPORTANT: Brain Fog

If they forget words mid-sentence or can't make a decision, it's not "lack of attention." Their processor has overheated. Ask simple questions (YES/NO). Give them time.

DO: Believe them.

When they say "I can't speak," it's not a metaphor. It's energy paralysis. Bring salty water (electrolytes). Text instead of calling (phone audio is draining).

AVOID: "You look fine."

This is one of the most hurtful phrases. Their illness is invisible. A compliment suggests the problem is in their head. Also avoid strong perfumes – they cause physical pain.

Emotional Support

A sick person's biggest fear is being a burden. Reassure them: "We are in this together", "You are not a problem for me." This heals the soul when the body refuses to cooperate.
